AVS - Missed Ride Report

Students approved by UCONN Center for Students with Disabilities to receive Accessible Vans Service (AVS) may use this form to report incidents of their regular weekly rides being missed or apparently unprovided. We will investigate and respond to all missed ride reports within two business days.
